Simply call Customer Service at (855) 249-0008 during normal business hours. Please be ready to provide your full name, address and phone number.
After this refund request is made within 30 days of purchase, we will gladly refund the full amount. You will not be eligible for benefits under the plan(s) or entitled to any benedfits from the plan(s).
